29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  On or about November 9, 2016, in the service building mezzanine area, a Delta Industrial table saw was used to rip sheets of plywood. The portion of the blade above the table was not guarded there by exposing employees to an amputation hazard.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  29 CFR 1910.178(l)(2)(ii) is applicable to shipyard employment by 29 CFR 1915.120 that incorporates the general industry requirements.    On or about November 9, 2016, Torresen Marine Inc., employees required to operate forklifts were not provided training on the safe operation of forklifts that included formal instruction, practical training and an evaluation of the operators performance in the workplace.    The Torresen Marine Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number 963812, citation number 1, item number 2 and was affirmed as a final order on May 9, 2014, with respect to a workplace located at Lakeshore Drive, Muskegon, Michigan 49441.
