29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Worksite - On or about November 9, 2016, employees were exposed to struck-by hazards to the eyes due to the employees removing old furniture, wood and loose debris from the building and then throwing this material out of the 3rd floor window without wearing safety glasses.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations: a) Worksite - On or prior to November 18, 2016, employees were exposed to fall and struck-by collapse hazards due to working and performing demolition operations such as removing walls in a structure that was in a poor deteriorated condition with interior parts of the building that had already collapsed. There was approximately 20 to 30 years of water damage due to the chimney and skylight areas leaking water. The employer did not have an engineering survey performed by a competent person prior to starting demolition operations.
