29 CFR 1910.147(e)(1): Before lockout tagout devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact:    On or about October 13, 2016, Line 2 Fryer, the employer did not ensure the valve component to the used oil return line was closed prior to removing lockout/tagout devices and start-up procedures.  Failure to do so allows caustic and water to accumulate in the piping system and while employee was engaged in purging lines, water flowed and mixed with hot grease in the decrumber which generated a volatile discharge of oil.  Employees were exposed the hazard of contact with hot grease, thermal and chemical burns.
