29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Rear-Line 60% J-Clip Fixture station no. 47: on or about December 13, 2016, operators assigned to insert their hands in the point of operation to manually assemble automotive seat components were exposed to caught in-between hazards likely to result in serious injury due to uninitiated cycling ("ghost cycling") of a j-clip positioning blade. The safeguarding provided was not adequately designed and constructed to prevent uninitiated operating cycles or to prevent operators from having their hands/fingers in the danger zone during uninitiated operating cycles. There is no abatement certification or documentation required for this item.
