29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response), or equivalently, a dose of fifty percent:    On or about November 7, 2016, the following employees were exposed to elevated noise levels while performing routine work activities in the Polyester Department and the employer did not administer a continuing, effective hearing conservation program where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels :    a) An employee using pneumatic tools to smooth edges of plastic injection molding pieces was exposed to an 8-hour TWA noise level of 88.55 dBA measured on the A scale with a dose of 81.8% during a 377 minute sampling period assuming zero exposure for the unsampled 103 minutes.    b) An employee using pneumatic tools to smooth edges of plastic injection molding pieces was exposed to an 8-hour TWA noise level of 92.03 dBA measured on the A scale with a dose of 132.5% during a 373 minute sampling period assuming zero exposure for the unsampled 107 minutes.    c) An employee using pneumatic tools to smooth edges of plastic injection molding pieces was exposed to an 8-hour TWA noise level of 92.5 dBA measured on the A scale with a dose of 142% during a 391 minute sampling period assuming zero exposure for the unsampled 89 minutes.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
