29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  Rear 53 North St., Shrewsbury, MA: On or about November 3, 2016, employees were building the roof system from the upper floor of the residence being constructed and were exposed to fall hazards during the following activities:  a) Falls in excess of 20 feet from the upper level to the ground below while setting the roof rafters.  b) Falls in excess of 10 feet to the next lower level while adjacent to a floor hole that ran the length of the upper floor while setting the roof rafters.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).  Andre Kassis DBA Kassis Custom Builders was previously cited for a violation of this occupational safety and health standard 29 CFR 1926 501B 13, which was contained in OSHA inspection number 941975 citation number 1, item number 2 and was affirmed as a final order on 10/28/2013, with respect to a workplace located at 14 Dupuis Ave., Worcester, MA.
