29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:     On or about 11/8/16 in Dock #2, East side of the production building; the employer allowed employees to set up and change the blades on the #1 & 2 Spack System Core Cutter machines without using machine-specific lockout/tagout procedures; thus exposing employees to potential laceration/amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about 11/8/16 in Dock #2, East side of the production building; the employer allowed employees to set up and change the blades on the #1 & 2 Spack System Core Cutter machines without conducting a periodic inspection of the energy control procedure; thus exposing employees to potential laceration/amputation hazards.
