29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about 11/9/16 at the Core and Panel machines; the employer allowed two employees covered by the lockout/tagout standards to clean the cutting blade at the packing section of the machine, and no periodic inspection had been conducted, as was witnessed that only one employee had his locks on both of the energy sources; thus exposing the other employee to potential laceration/amputation hazards.
