29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about November 10, 2016, and in times prior thereto, the tongue guard had been left off of a Baldor, bench grinder, located in back of the shop. This condition exposed workers to the hazard of being struck by pieces of the abrasive wheel should it shatter.
