29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a) At or near 464 Blue Barn Rd. Allentown, PA - on or about November 10, 2016, an employee was observed working on a tubular welded frame scaffold, approximately 18 feet above grade, where the working level consisting of nominal 10 inch planks was installed so the space between the adjacent units and the space between the platform and uprights was greater than one inch wide.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) At or near 464 Blue Barn Road in Allentown, PA - on or about November 10, 2016, an employee performed masonry on a scaffold platform greater than 18 feet from lower levels and was not protected from falling.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) At or near 464 Blue Barn Rd. Allentown, PA - on or about November 10, 2016, employees working from a tubular welded frame scaffold were using cross bracing as a means of access.
