29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, Building #5, Straightening Department: On or about November 10, 2016, there were unguarded rotating parts to the #3 Medart straightening machine. (b) Facility, Building #5, Straightening Department: On or about November 10, 2016, there were unguarded rotating parts to the #0 Medart straightening machine. (c) Facility, Building #5, Straightening Department: On or about November 10, 2016, there were unguarded rotating parts at the one end of the #2 Medart Plane. (d) Facility, Building #5, Rolling Mill: On or about November 10, 2016, there was an inadequately guarded coupler in that the rail that was being used for employee protection from the rotating coupler, was not secured and that the rail was only about 15 inches from the rotating coupler.
