29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:  a) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - An employees assembling wood boxes using nail guns was exposed to noise levels at a time weighted average (TWA) of 93.7 dBA, which exceeded the action level of 85 dBA. A noise training program was not instituted, on or about 11/15/16.  b) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - An employee assembling wood boxes using nail guns was exposed to noise levels at a time weighted average (TWA) of 93.4 dBA, which exceeded the action level of 85 dBA. A noise training program was not instituted, on or about 11/15/16.  c) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - An employee assembling wood boxes using nail guns was exposed to noise levels at a time weighted average (TWA) of 93.2 dBA, which exceeded the action level of 85 dBA. A noise training program was not instituted, on or about 11/15/16.  d) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - A temp employee assembling wood boxes using nail guns was exposed to an average noise level of 93.8 dBA, which exceeded the action level of 85 dBA. A noise training program was not instituted, on or about 11/15/16.  e) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - A temp employee assembling wood boxes using a staple gun was exposed to noise levels at a time weighted average (TWA) of 88.5 dBA, which exceeded the action level of 85 dBA. A noise training program was not instituted, on or about 11/15/16.  f) Atlanta Industrial Packaging, Inc. dba Reid Manufacturing, Norcross, GA - A temp employee assembling wood boxes using a nail gun was exposed to noise levels at a time weighted average (TWA) of 86.9 dBA, which exceeded the action level of 85 dBA.
