This requirement does not apply to the integral fuel tanks of motor vehicles.  On or about November 12, 2016, at the residential construction site located at Parkway Estates Subdivision in Blue Springs, Missouri: Employees were exposed to a fire hazard because a gas can containing flammable liquid was present and no fire extinguisher was available at the job site.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 12, 2016, at the jobsite located at Parkway Estates Subdivision in Blue Springs, Missouri:  Employees engaged in residential roofing activities were exposed to fall hazards of greater than 6 feet from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.
