29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 12, 2016, at the residential construction site at Parkway Estates Subdivision in Blue Springs, Missouri: Employees engaged in residential roofing activities were not protected from overhead hazards of material, equipment, or other employees dropping materials and striking employees below who were not wearing any means of head protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about November 12, 2016, at the residential construction site located at Parkway Estates Subdivision in Blue Springs, Missouri:  Employees engaged in roofing using a pneumatic nail gun were exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite. This requirement does not apply to the integral fuel tanks of motor vehicles.  On or about November 12, 2016, at the residential construction site located at Parkway Estates Subdivision in Blue Springs, Missouri: Employees were exposed to a fire hazard because a gas can containing flammable liquid was present and no fire extinguisher was available at the job site.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 12, 2016, at the jobsite located at Parkway Estates Subdivision in Blue Springs, Missouri:  Employees engaged in residential roofing activities were exposed to fall hazards of greater than 6 feet from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.
