29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about November 14, 2016, and times prior thereto, on a rooftop, employees were installing asphalt shingles using pneumatic nail guns without the use of eye protection, exposed to hazards of flying projectiles.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  On or about November 14, 2016 and times prior thereto, on an aluminum scaffold platform, employees installing asphalt roof shingles were using bundles of the shingles as base support for the front of the platform, which created a greater than 9 ft. fall to the ground below.

29 CFR 1926.452(n)(3): Ladders used to support step, platform, and trestle ladder scaffolds shall be placed, fastened, were not equipped with devices to prevent slipping:  On or about November 14, 2016 and times prior thereto, on a roof, where employees were installing asphalt shingles, the ladder used to support the work platform for the task involved was not fastened, or equipped with devices to prevent slipping. This condition exposed employee(s) to ladder displacement, which would create a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about November 14, 2016, and times prior thereto, at the jobsite, employees were ascending and descending from the rooftop on an aluminum extension ladder which did not extend at least three feet above the rooftop landing, exposing employees to fall hazards greater than 6 ft. to the ground below.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about November 14, 2016, and times prior thereto, at the jobsite, the employer did not ensure that a Louisville ladder with visible defects was not withdrawn from service until repaired. This condition exposed employees to fall hazards of greater than 9 feet to the ground below.
