29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Inside water tank, Field Club Road, Pittsburgh, PA 15238: On or about November 14, 2016, a swing stage scaffold being used by two employees to sandblast the interior of a water tank was not inspected by a competent person prior to each work shift. The swing stage scaffold was improperly rigged. One end of the swing stage scaffold collapsed seriously injuring an employee who was working on the scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards: a) Inside water tank, Field Club Road, Pittsburgh, PA 15238: On or about November 14, 2016, the employer did not ensure that each employee who was working on a swing stage scaffold was trained by a competent person to recognize the hazards associated with the type of scaffold being used and to recognize the procedure(s) to be used to minimize these hazards. Two employees were working off of a swing stage scaffold that was not rigged properly and the employees were not protected from falling by the use of personal fall arrest systems.

The swing stage scaffold was moved on or before November 12, 2016 and again on November 14, 2016. The swing stage scaffold was not rigged properly. One end of the swing stage scaffold collapsed seriously injuring an employee who was working on the scaffold.
