At the workplace, on or about 11/14/2016, the employer was not maintaining OSHA 300 and 300A forms for 2016.  A recordable injury had occurred at the facility on October 17, 2016.  Rod's Western Palace was previously cited for a violation on this occupational safety and health standard, which was contained in OSHA inspection number 1091660, citation number 2, item number 1, and was affirmed as final order on December 23, 2015, with respect to a workplace located at 3650 Indianola Avenue, Columbus, Ohio 43215.
