On or about November 2, 2016 and at times prior thereto, employees were using hand tools such as hammers and pneumatic nail guns without eye protection, exposed to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about November 2, 2016 and at times prior thereto, employees engaged in roofing construction at the Super 8, 6001 Rogers Avenue, Fort Smith, Arkansas, were on a 4/12 pitch roof without fall protection, 18 feet high, exposed to the hazard of falling to the ground below.

On or about November 2, 2016 and at times prior thereto, the ladder at the Motel 6 worksite, 6001 Rogers Avenue, Fort Smith, Arkansas had shingles placed on the first ladder rung and in front of the ladder exposing employees to trip hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about November 2, 2016 and at times prior thereto, at the Motel 6, 6001 Rogers Avenue, Fort Smith, Arkansas, employees were using a portable metal extension ladder to access the second story roof that did not extend at least three feet above the upper landing surface. This condition exposed employees to fall hazards.
