29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a) Unit 36, Floor in Front of Window- There was three nails protruding up one inch from the floor near where the ladder was being used, on or about November 11, 2016. b) Unit 36, Stairs to Second Floor - There was wood debris on the stairs and landing between the first and second floors, on or about November 11, 2016.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) Building 6, Unit 36, Between the First and Second Floors - There was no hand rail on the enclosed stairs between the first and second floors, on or about November 11, 2016. The stairs had 8 risers, a landing and then another 8 risers.
