Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded. On or about November 15, 2016, the light fixture installed on the coiler-oven machine was not guarded from accidental contact or breakage. This condition exposed employees to struck-by hazards.

This condition exposed employees to electrical hazards.
