29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe-boards. Note:  Toe-boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   On or about 14 November 2017, and at times prior thereto, employees accessing open-sided platforms were not provided fall protection guarding with either standard railing and toe-boards or equivalent acceptable fall protection means, exposing the employees to fall hazards in excess of 4 feet or more.  1) Employees were required to access the rooftops of Electrical Rooms monthly to replace air filters at a height of approximately 15 feet 7 inches from the concrete floor below. 2) Employees were required to access the top layer of stacked tubing within the Warehouse to lay dunnage at heights exceeding 4 feet and up to and including approximately 18 feet high above the concrete floor below or other stack surfaces.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about 14 November 2016, and at times prior thereto, employees were required to bypass fixed barrier guards to perform work or minor maintenance during the normal operation of the line, exposing the employees to the hazard of being struck by the product or caught in the moving conveyor system. 1) At the Bundler area, an employee was required to bypass a fixed barrier fence in order to spray pipe ends. 2) At the Strap Conveyor zone, an employee was required to bypass a fixed barrier fence in order to clean up scarf or re-seat a pipe.

On or about 14 November 2016, and at times prior thereto, a Dayton 6-inch pedestal bench grinder was not bolted to the floor, exposing employees using the tool to the hazard of being struck by the grinder.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  On or about 14 November 2016, and at times prior thereto, the employer was utilizing Lockout-Tagout Procedures for maintenance personnel that had not been reviewed at least annually, exposing the employees to the various energy sources for the equipment. 1) ID# Finish Section 3, Lock-out procedure for MAIR Section 3 Flush-Out had a last revision date of 02/19/15, exposing the maintenance employees to the energy sources of mechanical, air, and electrical. 2) ID# Finish Section 9, Lock-out procedure for MAIR Section 9 API Bundler had a last revision date of 02/20/15, exposing the maintenance employees to the energy sources of mechanical, air, and electrical.

29 CFR 1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months.  On or about 14 November 2016, and at times prior thereto, an alloy steel chain sling set was used within the facility to lift and move bundles of pipe that had not been inspected at least once within 12 months, exposing the employees using the sling set to the hazard of being struck by a load up to approximately 10,000 pounds.
