29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At 24900 Perdido Beach Blvd., Orange Beach, Alabama: on or about November 15, 2016 and at times prior; the employer allowed his employees to work on a high slope roof without fall protection exposing them to a fall hazard of 10 feet 8 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 24900 Perdido Beach Blvd., Orange Beach, Alabama: on or about November 15, 2016 and at times prior; the employer failed to ensure an extension ladder used by employees to gain access to the roof had its rails extend at least three feet above the landing area exposing his employees to a fall hazard of 10 feet 8 inches to the ground below.
