29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section. a. On or before November 15, 2016, where employees were working from scaffolding installing siding to the second floor of the house, the scaffolding was not designed by a qualified person, thereby exposing the employees to a 26 foot fall hazard.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system. a. On or before November 15, 2016, where employees were working from ladder jack scaffolding installing siding to the second floor of the house, the employees were not protected from falls, thereby being exposed to a 26 foot fall hazard.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: 29 CFR 1926.454(a)(2): The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used. a. On or before November 15, 2016, where employees were working from ladder jack scaffolding installing siding to the second floor of the house, the employees had not received training for erecting, maintaining, disassembling, or fall protection, thereby exposing the employees to a 26 foot fall hazard.
