(a) location; Near 131 Water Street (Water Street Bridge), Danvers, MA, 01923. On or about November 10, 2016, A lifesaving skiff was not readily available for use while an employee was working on the outside railing of a bridge were no fall protection was worn. Skiff was on the road side of the bridge stored on top of a work skiff and chain locked to the work skiff.

On or about November 10, 2016, An employee was not protected from falling while working from an elevated work surface greater than ten feet above water and rocks. The employee was dismantling planking above the water and rocks were no fall protection was worn.
