29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) Lawson Brothers Construction, Inc. on or about November 15, 2016 at the address of 353 Wildbrook Ln. Lima, Ohio: The employer failed to ensure fiber glass A-framed self-supporting ladders were opened during use. Three A-Framed ladders were observed in the closed position leaning against the structure while employees used the ladder to access the roof level, approximately eight feet. Employees were exposed to fall hazards due to the improper use of these ladders.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a)Lawson Brothers Construction, Inc. on or about November 15, 2016 at the address of 353 Wildbrook Ln. Lima, Ohio: The employer failed to ensure employees were protected from falls of approximately eight feet while working on the roof of a residential home while installing protective paper covering. Lawson Brothers Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(13) which was contained in OSHA Inspection Number 1060743, citation number 1, item number 2 and was affirmed as a final order on July 4, 2015, with respect to a workplace located at 7560 Lehman Road Delphos, Ohio 45833.
