29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be nescessary to comply with the standards set forth in 29 CFR 1926 as evidenced by the violation(s) noted during the course of the inspection:  a)  On November 15, 2016 at the address of 517 Huron Street, Huron, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a)  On November 15, 2016, at the address of 517 Huron Street, Huron, OH: the employer did not ensure an employee was protected from fall hazards while working from a ladder jack scaffold.  The employee was exposed to an 18 feet fall to the ground below during roofing operations of a residential structure. No form of fall protection was in place.
