29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   a. G&L Siding and Roofing LLC jobsite located at 215 W. Center Street, Bucyrus, Ohio:  On or about November 15, 2016, the employer did not ensure employees were protected from 20.4 foot fall hazards when working on a ladder jack scaffold to install siding on the North side of the structure.     b. Center Street, Bucyrus, Ohio:  On or about November 15, 2016, the employer did not ensure employees were protected from 20.4 foot fall hazards when working on a ladder jack scaffold to install siding on the East side of the structure.

29 CFR 1926.452(k)(3): The ladder jack was not designed and constructed that it will bear on the side rails and ladder rungs or on the ladder rungs alone. If bearing on rungs only, the bearing area shall include a length of at least 10 inches (25.4 cm) on each rung.  a. G&L Siding and Roofing LLC jobsite located at 215 W. Center Street, Bucyrus, Ohio:  On or about November 15, 2016, the employer did not ensure the south end of the ladder jack platform rested on the rungs of the ladder.  The platform was attached to the house's aluminum antenna tower and secured to the extension ladder with a ratchet strap.

29 CFR 1926.1053(b)(7): Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.    a. G&L Siding and Roofing LLC jobsite located at 215 W. Center Street, Bucyrus, Ohio:  On or about November 15, 2016, the employer did not ensure  extension ladders used as part of a ladder jack scaffold system were erected on a slip resistant surface.
