29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Jobsite, 271 Field Club Circle, McKees Rocks, PA:  On or about November 16, 2016, 6 employees were installing shingles on a residential roof without fall protection.  The fall distance ranged from 28 to 34 feet to the ground.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 271 Field Club Circle, McKees Rocks, PA:  On or about November 16, 2016, 6 employees were working on a residential roof above 6 feet and were not protecting from falling.  The employer had no written documentation of providing fall protection training to employees.
