29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 15, 2016, employees were exposed to struck-by and tripping hazards when the following were not guarded or were not adequately guarded: a. ProEco line stacker:  Employees walk and work near and in the stacker which has no guards to prevent access during operation.   b. Brite line stacker:  Employees walk and work near the stacker which has no guards to prevent access during operation. c. ProEco chain conveyor platform:  Employees walk and work on and near the chains and pallets on the chains which are tripping hazards.  They are struck-by hazards if someone accesses the controls to move them. d. ProEco chain conveyor platform back of the stacker:  Employees walk and work on and near the chains and pallets on the chains which are tripping hazards.  They are struck-by hazards if someone accesses the controls to move them. e. Brite line chain conveyor platform:  Employees walk and work on and near the chains and pallets on the chains which are tripping hazards.  They are struck-by hazards if someone accesses the control to move them.  f. Brite line chain conveyor platform back of the stacker:  Employees walk and work on and near the chains and pallets on the chains which are tripping hazards.  They are struck-by hazards if someone accesses the controls to move them. g. ProEco packing conveyors:  Employees walk, lie down, and kneel on the conveyor while packaging pallets which are tripping hazards.  They are struck-by hazards if someone accesses the controls to move them. h. Brite line packing conveyors:  Employees walk, lie down, and kneel on the conveyor while packaging pallets which are tripping hazards.  They are struck-by hazards if someone accesses the controls to move them.   Employees were exposed to caught-in and struck-by hazards when the following were not guarded: i. The chain/sprocket covers were missing/broken on three of the eight chain conveyors on the pallet load end of the Brite line (behind the stacker). j. There are no guards over the chain conveyor chains/sprockets on the packaging side of the Brite line stacker.  Employee walks along the yellow cover at the end of the chains approximately once a month when the line is full. k. There are no guards over the chain conveyor chains/sprockets on the packaging side of the ProEco line stackers.  Employee stands on the chains near the sprockets to put tags on the skids when the line is full.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Employees walk and work in the area. a. Employees stand next to and reach around the guide/forming rollers and scarfing blade at the scarfing station b. Employees work/walk near and make adjustments to guide/forming rollers at the entry to the cutting blade c. Employees work/walk near and make adjustments to guide/forming rollers between scarfing and cutting to length d.
