29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 1213 Walnut Street, Philadelphia, PA 19107, From 8th Floor to 22nd Floor - The employer did not provide a fall protection system around the floor holes in the walking/working surfaces thus exposing the employees to fall hazards. The employees accessed levels above and below the walking/working surfaces by use of ladders. This condition was observed on or about November 1, 2016.
