29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:     On or about November 15, 2016, at 2500 Hametown Rd., in Copley, OH, while attempting to replace a 3 inch leaking gas line in an excavation of  6 - 7.5 feet, employees were not provided with a safe way to exit the excavation.

29 CFR 1926.651(g)(1)(iii): Adequate precautions were not taken such as providing ventilation, to prevent employee exposure to an atmosphere containing a concentration of a flammable gas in excess of 20 percent of the lower flammable limit of the gas:     On or about November 15, 2016, workers in the process of using a hand grinder to cut a 2 3/8 inch steel tube were exposed to hazards created by sparks, which ignited the flammable gases. A heat source was introduced into the work area and an explosion occurred resulting in the hospitalization of two workers.
