29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems: Building Q2, On or about November 16, 2016, employees were exposed to a fall over 6 feet in height to the ground level below, while removing material from a roof pitch 4:12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Building Q2, On or about November 16, 2016, employees were exposed to a fall hazard greater than 6 feet, while using a 12 foot ladder to access the building roof which had the side rails approximately 4 inches beyond the landing, to install roof shingles.
