29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a) On November 16, 2016, the floor area of D-123 Grinding Machine unloading area was slippery with a coating of machine fluid and metal surface protectant.

29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On November 16, 2016, the D-123 Through Feed Centerless Grinder Operator was not wearing personal protective equipment to prevent skin contact with the machining fluid and metal surface preservative for his legs, torso and arms.

29 CFR 1910.147(c)(7)(iv): The employer shall certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employee's name and dates of training. a) The D-123 Through Feed Grinder Operator energy control training was not documented. The D-123 Through Feed Grinder Operator uses energy control devices to de-energize his equipment before performing preventative maintenance, setup, cleaning and adjusting activities.
