OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that compressed air was distributed throughout the shop floor by use of Polyvinyl Chloride (PVC) pipes exposing employees to explosion hazards and sharp flying projectiles in the air due to a rupture in the line.  On or about 16 November 2016, and at times prior thereto, polyvinyl chloride (PVC) piping was used throughout the shop floor for compressed air in which the system was charged to approximately 110 psig, exposing the employees within the shop to the hazard of being struck by flying projectiles.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.I.  On or about 16 November 2016, and at times prior thereto, employees used compressed air for cleaning of workstations at various machines and work tables in which the air pressure was not reduced to at least 30 psi, exposing the employees to the hazard of being struck by flying debris or to an air embolism.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about 16 November 2016, and at times prior thereto, the Yama Seiki BM-1020 CNC Mill Machine was used with the interlock bypassed and the door of the machine open while running, exposing the Operator to the hazard of being struck by or caught in rotating parts.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.  On or about 16 November 2016, and at times prior thereto, a pedestal bench grinder was utilized on the shop floor with each tongue guard adjusted beyond one fourth inch, exposing employees using the grinder to a struck by hazard from the wheel disintegrating.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  On or about 16 November 2016, and at times prior thereto, unused breaker slots within breaker panels were not effectively closed, exposing employees accessing the panel to an electrical hazard of approximately 220 volts. 1) Breaker Panel near junction box labeled P2 7-8-9 had missing breaker slots 25 through 27 and slot 36. 2) Breaker Panel near transformer had missing slots 1 and 2, plus 17, 19, and 21 which are side by side. 3) Breaker Panel N1 had missing slots 12, 14, and 16 which are side by side. 4) Breaker Panel N-2 had missing slots 4 and 29, 31, 33, 35 which are side by side.
