29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 16, 2016, on the roof of a restaurant, where employees were exposed to fall hazards exceeding 6 feet while performing roof decking activities without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about November 16, 2016, on the southeast side of the building, where employees were exposed to a fall hazard when using an extension ladder to access the roof without extending the ladder 3 feet beyond the landing of the roof.
