29 CFR 1910.165(e): Manually operated actuation devices for use in conjunction with employee alarms were not unobstructed, conspicuous and readily accessible.  a) At the BIA, Western Nevada Agency Reception Area, the manual pull fire alarm was obstructed by plants and furniture.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.  a) At the BIA, Probate Office, trash can and file cabinet was obstructing the exit access  b) At the BIA, Realty Office, cart loaded with files was obstructing the exit access, thereby reducing it to 19 inches wide.  The Bureau of Indian Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.36(g)(2),which was contained in OSHA inspection number 908527, citation number 1, item number 1, and was affirmed as a final order on 7/29/2013, with respect to a workplace located at Bureau of Indian Affairs, 711 State Street, Nixon, NV 89424.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  (a) At the Bureau of Indian Affairs, Western Nevada Agency, Supply room, a re-locatable power tap was used to plug-in an Absocold refrigerator, a Continental Microwave and a Fellones Shredder.  (b) At the Bureau of Indian Affairs, OST Office, a re-locatable power tap was used to plug-in an Emerson Microwave oven, exposing employees to electrical shock.  The Bureau of Indian Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2),which was contained in OSHA inspection number 927270, citation number 1, item number 3, and was affirmed as a final order on 7/29/2013, with respect to a workplace located at Bureau of Indian Affairs, Rick Lupe Fire Management Center, Whiteriver, AZ 85941.
