(b)  On or about November 16, 2016, and times prior thereto, in the Sheet Room, guarding was not provided on the 350 ton Cincinnati hydraulic press brake, s/n 54694, to protect employees performing sheet metal forming tasks from inadvertent contact with the point of operation.        Employees were exposed to struck-by and crushing hazards.
