29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation: On or about October 12, 2016, and times prior thereto, in the Bar Room, guarding was not provided on the Marvel 2150M-60 band saw to protect employees from inadvertent contact with pinch points. An accident occurred when an employee contacted the pinch point between the closing vise clamp and metal bar while performing metal bar cutting tasks. Employee was exposed to crushing hazards.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about November 1, 2016 and times prior thereto, in the Sheet Room, guarding was not provided on the 250 ton Cincinnati hydraulic press brake, s/n 54604. An accident occurred when an employee performing sheet metal forming tasks contacted the point of operation. (b) On or about November 16, 2016, and times prior thereto, in the Sheet Room, guarding was not provided on the 350 ton Cincinnati hydraulic press brake, s/n 54694, to protect employees performing sheet metal forming tasks from inadvertent contact with the point of operation. Employees were exposed to struck-by and crushing hazards.
