29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At the jobsite, employees operating Hitachi nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. At the jobsite, employees performing decking activities on the roof of a two story residence were not provided fall protection and exposed to an unprotected fall hazard at a height of over 12 feet to the ground below.
