29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Walter Construction LTD at 7600 E Oxford Ave, Denver, CO 80237:  On and before November 10th, 2016, the employer did not ensure that employees working on a two-story single family residence with a 5/12 roof pitch were protected from falling.  Employees were preparing the upper roof for shingling without the use of fall protection.  This condition exposed the employees to fall hazards of up to approximately 18 feet.
