On or about November 17, 2016, employees utilized an Atsco Horizontal Grinder with a type 27 wheel that did not have guarding in place while grinding castings. c. On or about November 17, 2016, employees utilized a Henry 65 Series Vertical Grinder with types 6 and 11 wheels without guarding in place while grinding castings.
