29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Jobsite: The company did not use safe and acceptable means to determine the exact location of a gas service stub. The excavator bucket contacted the stub and broke the gas main which resulted in a large release of natural gas.

29 CFR 1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, such as in excavations in landfill areas or excavations in areas where hazardous substances are stored nearby, the atmosphere in the excavation was not tested before employees entered excavations greater than 4 feet (1.22m) in depth: Jobsite: An employee entered a trench that was more than 4 feet deep following the rupture of a gas main. The atmosphere in the trench was not tested before the employee entered and the employee was overcome by the gas. Air monitoring was also not performed before additional employees entered the trench to rescue the 'downed' employee.

The employee was overcome by natural gas and had to be rescued by his co-workers who were also inadequately protected from the gas.
