29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  On or about November 17, 2016, employees were exposed to a fall hazard of greater than 6 feet to the ground level below, while they attached shingles to the roof, without using a personal fall arrest system, fall protection equipment, or an alternative fall protection plan.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 20, 2016, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while attaching underlayment to the roof, without being trained to recognize the hazards of a fall.
