29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)  Located along the front area of the production building structure, employees were observed working or gaining access to the building structure from a supported scaffold platform raised on the elevated forks of a JLG rough terrain forklift, on which the platform was not protected by standard guardrails or equivalent and the employees were not utilizing personal fall protection system equipment and were exposed to fall potentials in excess of 10 feet.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  Located along the front area of the production building structure, employees were observed performing roof deck sheet metal  installation associated operations without conventional fall protection, and were exposed to the unprotected side or edge of the roof eave with the fall potentials in excess of 10 feet.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a)  Located along the front area of the production building structure, employees were observed performing roof deck sheet metal  installation associated operations from the upper roof deck of the structure which had an unprotected side or edge along the roof eave line. As a part of this work operation employees were utilizing a harness with a dorsally attached lifeline, on which the lower end of lifeline was wrapped and anchored around a large duct system which entered between the upper and lower roof lines. The personal fall arrest lifeline system was observed in use rigged and used in a manner which would allow the user to fall from the upper roof and strike the lower roof deck.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
