29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the job site, Lot 293, on or about November 17, 2016, four employees were observed working from the roof installing roof shingles onto the roof deck without any means of fall protection, which exposed employees up to 19 feet fall hazards.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:     a)  At the job site, Lot 293, on or about November 17, 2016, an employee  ascending a 28 foot aluminum extension ladder did not use at least one hand to grasp the ladder, exposing him up to an 19-foot fall hazard.                                                                 b)  At the job site, Lot 293, on or about November 17, 2016, an employee descending a 28 foot aluminum extension ladder did not use at least one hand to grasp the ladder, exposing him up to a 16-foot fall hazard.
