29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace. On or about November 16, 2016, throughout the facility, the employer allowed an employee to wear a Bullard 88VX Supplied-Air Respirator (SAR) blasting hood to blast housing components of motors and generators, without ensuring the employee was medically evaluated prior to allowing use of the respirator.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use. On or about November 16, 2016 inside the warehouse, the employer allowed employees to walk and work around pressurized cylinder tanks without having valve covering caps. a. At the entrance/exit bay door area, three (3) pressurized Oxygen tanks were without valve cover caps. b. At the entrance/exit bay door area, two (2) pressurized Acetylene (flammable gas) tanks were without valve cover caps.
