29 CFR 1926.754(a): Structural stability was not maintained at all times during the erection process: a. Weaving Room- Employees working from and below loom J117 gantry extension where not protected from fall and struck by hazards where structural stability of the gantry extension was not maintained. On or about November 10, 2016 two employees sustained injuries after a portion of the gantry extension collapsed during construction causing an employee to fall approximately 9.5 feet from the gantry extension and a second employee was struck in the head by a piece of floor decking approximately 42 inches by 39 inches that fell from the gantry extension.

29 CFR 1926.754(e)(5)(i): Metal decking was not laid tightly and immediately secured upon placement to prevent accidental movement or displacement: a. Weaving Room- Employees working from and below loom J117 gantry extension where not protected from fall and struck by hazards where metal decking was not immediately secured upon placement. On or about November 10, 2016 two employees sustained injuries when two pieces of unsecured metal decking, approximately 42 inches by 39 inches, collapsed during construction causing an employee to fall approximately 9.5 feet from the gantry extension and a second employee was struck in the head by one of the pieces of floor decking that fell from the gantry extension.
