In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: (a) On November 15, 2016, an unprotected yellow extension cord approximately 50 feet long was passed through the closed and locked front double doors of Red Lion United Methodist Church to provide power for the sub-contractor's tools which were used to cut and install new siding on the church steeple. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: (a) On November 15, 2016, at approximately 9:00 AM, an employee who was installing siding on the church steeple and standing on the church roof, was wearing a fall protection harness that was tied off to the Genie Z-80 Lift platform railing. (b) On November 15, 2016, at approximately 2:30 PM, two employees who were installing siding on the church steeple and standing on the church roof, were wearing fall protection harnesses that were tied off to the Genie Z-80 Lift platform railing. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
