29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  On or about November 17th, 2016, at the jobsite located at 6900 NW 77th Ter. Medley, FL 33166, the employer did not provide the appropriate gloves (butyl or nitrile) to the employees who wore disposable vinyl gloves when using epoxy substances (Nonyl-phenol) and concrete in the casting operations.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    On or about November 17th, 2016, at the jobsite located at 6900 NW 77th Ter. Medley, FL 33166, the employer did not provide employees who use 3M filtering piece-face N-95 while performing mixing concrete and casting operations, with information and training in the use and care of the respirators.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about November 17th, 2016, at the jobsite located at 6900 NW 77th Ter. Medley, FL 33166, the employer did not provide effective training to employees who were exposed to hazardous chemicals such as, but not limited to, epoxy substances (Nonyl-phenol) and concrete while performing casting operations.  Employees were not familiar with the Globally Harmonized System of Classification and Labeling of Chemicals with pictograms or representations as a part of the new SDS and label format.
