29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about November 18, 2016, employees were exposed to a being struck by hazard.  The employees were framing a newly constructed residential home located in Columbia Station, Ohio 44028.  The employees were working in areas with the possibility of head injury without wearing protective helmets.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.      On or about November 18, 2016, employees were exposed to a fall of approximately 14 feet.  The employees were framing a newly constructed residential home located in Columbia Station, Ohio 44028.  The employees were working on all areas of the 2nd floor. There were several window openings that were not protected by a guard rail system nor were the employees wearing fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about November 18, 2016, employees were exposed to a fall of approximately 20 feet.  The employees were framing a newly constructed residential home located in Columbia Station, Ohio 44028.  The employees were working on the 2nd level without using any form of fall protection.    Stec Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1072760 citation number 1,item number 2 and was affirmed as a final order on 7/23/15, with respect to a workplace located in Strongsville, Ohio 44136.
