B) East side of residence, on or about November 18, 2016, employees had to get near the open sided edge of the second floor to retrieve material, exposing employees to a fall hazard of approximately 16 feet.

The ladder was not long enough to extend three feet above the landing area, exposing employees to fall hazard of up o 15 feet.
