29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     (a) Jobsite, Mini 10th Floor, Near The Top Of The Stairs:  On or about November 17, 2016, employees would walk by an opening that only had a top rail installed for fall protection.  There was no mid-rail and the fall hazard was about 13 feet to a concrete floor.     (b)  Jobsite, Restaurant Area:  On or about November 17, 2016, an employee was working (carrying debris) next to a pit that was about 12 feet in depth.  The only fall protection was one strip of yellow caution tape.  The pit was about 6 feet in width and about 12 feet in length.
